We like to ensure the referral process is efficient, rapid and reliable. In order to make the service quick and efficient you can use our on-line referral form to be which will be emailed directly to us.

Alternatively you can print out the downloadable PDF referral form and fax or post it to us.

We ask you to kindly forward up to date contact details for your patients with e-mail addresses for you and them, where possible to allow for better communication.

We accept referrals for:
• Dental Implants and related prosthodontics and restorative work
• Endodontics
• Periodontics
• Oral Surgery
• Treatment with inhalation sedation
• Dental Hygiene Services
• X-rays 2D Panoramics or 3D CBCT
